Ohio University Career-Technical Articulation Verification Form

Students who successfully complete specified high school technical programs may be eligible to have technical credit transfer to Ohio public colleges and universities. Only students who did not consent to receive transfer credit during their exam completion must complete the following Career-Technical Articulation Verification (CTAV) request form.

By completing the request forms you are giving Ohio University Undergraduate Admissions permission to retrieve the Credit Availability Report for the Higher Education Information portal to confirm and award credit for completed career technical program(s) and/or coursework on your behalf.
I understand that (1) I have the right not to consent to the release of my education records; (2) I have the right to review such records upon request; (3) and that this consent shall remain in effect until revoked by me, in writing, and delivered to Ohio University, but that any such revocation shall not affect disclosures previously made by Ohio University prior to the receipt of any such written revocation.
